General treatment principles of Chinese medicine for hypochondrium pain

The article “The pain of hypochondrium in Chinese medicine” describes the causes of pain of hypochondrium and differentiation between syndromes of pain of hypochondrium. This article is describing the general treatment principles for improving pain of hypochondrium.

In traditional Chinese medicine, there are two basic treatment principles for pain. Blocking causes pain, so unblocking removes pain. Losing nourishment causes pain, so nourishing removes pain.

If the pain of hypochondrium is an excess syndrome, the main treatment method is to remove pathogenic factors and unblock liver Qi. The choosing and dosage of herbs for removing pathogenic factors should avoid the side effect that hurts Yang.

When the pain of hypochondrium is a deficiency syndrome, the main treatment method is to nourish Yin and blood, and soften liver. And herbs that unblocks liver and regulates Qi may also be added. However, most of the herbs that unblocks liver and regulates Qi is pungent in taste, and warm and dry in nature, which may hurt liver Yin or even foster heat’s transforming into fire. Therefore, the herbs that is pungent in taste and neutral in nature like Xiang Fu and Lyu E Mei, or the herbs that is warm but mild in nature like Fo Shou and Su Geng are better for unblocking liver and regulating Qi for a deficiency syndrome.

For a long-term syndrome that mixes deficiency and excess, the treatment should attach importance to improve deficiency, while the use of the herbs of activating blood and removing stasis should be mild for avoiding the side effect which hurts Qi, blood and Yin.

The recipes that relieve pain can be used as an assisting method. The recipe Jin Ling Zi San which can unblock liver and remove heat, and activate blood and relieve pain, or the recipe Shao Yao Gan Cao Tang which can nourish Yin, soften liver, moderate acute symptoms and relieve pain, is helpful to relieve pain of hypochondrium.
